The Kentucky State Police in Ashland is investigating an escape from the Transitions Incorporated on 29th Street Ashland, KY. The escape occurred on 4-29-2012 around 10:22 PM. The person was discovered missing during an inmate count of personnel at the facility. This count was prompted after a worker heard an audible alarm indicating a door had been opened.

The person is Michael Bernard, age 28, white male, described as 5-11, 125 pounds, brown eyes and brown hair. Subject has history of drug abuse and has prior home addresses in Columbia, KY and Greensburg, KY.

The case is still under investigation by Trooper Craig Morella.
