Sylvester Chase, 78, of Hopkinsville, formerly of Park City, died Thursday, June 21, 2012 at Western State Nursing Facility in Hopkinsville.

He was preceded in death by his parents, Cleo and Moten Chase; two sisters, Katherine Chase and Anna Lou Curry.

He is survived by two brothers, William Curry (Juanita) of Louisville and Jerry Curry (Yvonne) of New Mexico.

The funeral will be held 2:00 p.m. Monday at Zion Hill Baptist Church in Park City.  Burial will follow in the church cemetery. Visitation after 11:00 a.m. Monday at the church .
